Job summary

Matrix Medical Network provides in-home comprehensive health assessments to seniors and at‑risk individuals. As an NP, you review medical history, current health, medications, and risks during visits that may last up to one hour.

You will administer tests, coordinate with physicians and care teams, and deliver education and follow‑up in patients’ homes or SNFs, enhancing care quality and closing gaps.

Qualifications

• Must hold an NP license in the state of practice with prescriptive authority if needed.
• Board certification in Adult/Family/AG-Primary Care or similar.
• Current BLS/ACLS/CPR certification and valid driver’s license.

Responsibilities

• Conduct adult/geriatric assessments including medical history and follow‑up.
• Administer point-of-care and lab tests as needed.
• Collaborate with PCPs, case managers and family members for education and care plans.
• Provide services at the member’s home or in facilities.
