Job summary

Matrix Medical Network is seeking Nurse Practitioners to perform in-home comprehensive health assessments for older adults and at‑risk individuals. Each visit lasts up to one hour, reviewing medical history, medications, social factors, and risks to identify gaps in care.

You will collaborate with physicians and care teams, travel to members' homes or SNFs, and enjoy flexible scheduling, competitive pay, mileage reimbursement, and opportunities to advance to full-time roles.

Qualifications

• Master’s degree in nursing or commensurate experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority if needed.
• Board certification in Adult/Adult Gerontology Primary Care, Family, Emergency or related specialties.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, referrals and documentation.
• Administer and order point-of-care and lab-based tests as needed.
• Collaborate with physicians, case managers, social workers, family members and caregivers.
• Coordinate with PCP on education and follow-up.
• Provide services at members' residences (homes or SNFs).
