Job summary

Matrix Medical Network seeks a Nurse Practitioner to perform comprehensive in-home health assessments, reviewing medical history, medications, and social factors to identify care gaps. This role emphasizes high-quality, person-centered care in patients' homes or SNFs.

Providers administer tests as needed, collaborate with primary care physicians and care teams, and support targeted health improvements through individualized plans and follow-up. Flexible scheduling applies.

Qualifications

Master’s degree required or commensurate NP experience.
Current NP licensure in state of practice with prescriptive authority.
Board certification in Adult/Adult Gerontology/Family or Emergency care.
Current BLS/ACLS certification.
1 year NP experience preferred; new grads welcome.
Valid state driver’s license and automobile insurance.
Familiarity with health/medical record software and strong communication skills.

Responsibilities

Conduct adult/geriatric assessments including medical history, diagnoses, and treatment plans.
Administer and order point-of-care and lab-based testing as needed.
Collaborate with physicians, case managers, social workers, and caregivers.
Coordinate with PCP for education and follow-up care.
