Job summary

Matrix Medical Network seeks a Nurse Practitioner to perform comprehensive in-home health assessments for seniors. You will review medical history, medications, and social factors, and collaborate with physicians to close care gaps. This role emphasizes compassionate, patient-centered care delivered in the patient's home or SNFs.

Qualifications

• NP licensure in state of practice with prescriptive authority.
• Current NP board certification (AANP/AACN/ANCC) in adult-gerontology or family.
• Valid BLS/ACLS or CPR certification; min 1 year NP experience preferred.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and documentation per Matrix guidelines.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with PCPs, physicians, case managers, social workers, and family members for patient education and follow-up.
• Provide services at member's residence which may be homes or SNFs.
• Maintain strong communication and ensure proper documentation.
