Job summary

Health Connect America seeks a Family Development Specialist to support clients and families by implementing treatment plans and providing therapeutic interventions in home and community settings. You will coordinate with the treatment team, maintain documentation, and recruit referrals to sustain a full caseload.

This role emphasizes compliance with COA standards and state regulations, working within the homes of clients and linking families to needed services.

Qualifications

• Bachelor’s degree in a human services discipline is required.
• Minimum of two years experience working with children and/or families (internships and volunteer work may count).

Responsibilities

• Develop individualized treatment plans and coordinate care with the treatment team and Clinical Supervisor.
• Provide individual, group, and family therapeutic interventions, as well as skill-building per the Treatment Plan.
• Attend and participate in weekly Treatment Team meetings.
• Link clients and families with services and resources identified in the treatment plan.
• Serve as liaison with other professionals, agencies, and community resources.
• Maintain accurate documentation of client interactions and progress notes in compliance with policy and regulatory standards.
• Actively recruit new referrals to maintain a full caseload.
• Provide 24/7 on-call support as required by the program.
• Ensure compliance with state regulatory bodies and COA standards.
• Participate in ongoing training and professional development.
