Job summary

Health Connect America is seeking a Family Development Specialist to support clients and families by developing treatment plans, providing therapeutic interventions, and linking them to needed services in home, community, and office settings.

You will collaborate with the treatment team, maintain accurate documentation, recruit referrals, and participate in ongoing training to ensure compliance with COA standards and regulatory requirements.

Qualifications

• Bachelor’s degree in a human services discipline is required.
• Minimum of two years experience working with children and/or families.

Responsibilities

• Develop individualized treatment plans and coordinate care with the treatment team.
• Provide individual, group, and family therapeutic interventions and skill-building.
• Attend weekly Treatment Team meetings and recruit new referrals.
• Maintain accurate documentation of client interactions and progress notes.
• Provide 24/7 on-call support as required and ensure compliance with COA standards.
