Job summary

Health Connect America is seeking a Family Development Specialist to support families in their homes and communities. You will develop and implement treatment plans, provide therapy, and coordinate services with a multidisciplinary team to meet client needs.

The role involves clinical interventions, documentation, on-call support, and active referral development, all while ensuring regulatory compliance and quality service delivery.

Qualifications

• Bachelor’s degree in a human services discipline is required.
• Minimum of two years experience working with children and/or families, including internships or volunteer work.

Responsibilities

• Develop individualized treatment plans and coordinate care with input from the treatment team and Clinical Supervisor.
• Provide individual, group, and family therapeutic interventions and skill-building.
• Attend weekly Treatment Team meetings.
• Link clients and families with services and resources identified in the treatment plan.
• Serve as liaison with professionals, agencies, and community resources to meet client needs.
• Maintain accurate documentation of client interactions and treatment plans in line with policy and regulatory standards.
• Actively recruit new referrals to maintain a full caseload.
• Provide 24/7 on-call support as required by the program.
• Ensure compliance with state regulations and COA standards.
• Participate in ongoing training to stay current with best practices.
