Job summary

Health Connect America is seeking a Family Development Specialist to support children, families, and adults by developing and implementing treatment plans, coordinating care, and linking clients to community services in home and community settings.

You will work with the treatment team, maintain thorough documentation, recruit referrals to sustain caseloads, and participate in weekly meetings, while complying with COA standards and state regulations.

Qualifications

• Bachelor's degree in a human services discipline is required.
• Minimum of two years of experience working with children and/or families.
• Experience in home- and community-based settings is preferred.

Responsibilities

• Develop individualized treatment plans and coordinate care with the team.
• Provide individual, group, and family therapy as outlined in the plan.
• Attend weekly Treatment Team meetings.
• Link clients and families with services identified in the plan.
• Serve as liaison with professionals and agencies to meet client needs.
• Maintain accurate documentation of client interactions and progress.
• Actively recruit new referrals to maintain full caseload.
• Provide 24/7 on-call support as required.
• Ensure compliance with state regulations and COA standards.
• Participate in ongoing training to stay current with best practices.
