Job summary

Health Connect America is seeking a Family Development Specialist to support clients and families by developing and implementing treatment plans and linking them to services. This is a community-based role where you will be working within the homes of our clients.

You will provide individual, group, and family therapeutic interventions while maintaining thorough documentation and actively recruiting new referrals, with a focus on regulatory compliance and best practices.

Qualifications

• Bachelor's degree in a human services discipline is required.
• Minimum of two years experience working with children and/or families (may include internships/volunteer work).

Responsibilities

• Develop individualized treatment plans and coordinate care for clients and families with input from the treatment team and the Clinical Supervisor.
• Provide individual, group, and family therapeutic interventions, as well as skill-building, outlined in the Treatment Plan.
• Attend and actively participate in weekly Treatment Team meetings.
• Link clients and families with specific services and resources as identified in the treatment plan.
• Serve as a liaison with other professionals, agencies, and community resources related to the client, ensuring needs are met comprehensively.
• Maintain accurate and timely documentation of all client interactions, treatment plans, and progress notes in compliance with policy and regulatory standards.
• Actively recruit new referrals to maintain a full caseload.
• Provide 24/7 on-call support for clients as required by the program.
• Ensure compliance with state regulatory bodies and COA standards.
• Participate in ongoing training to stay current with best practices for quality service delivery.
