Job summary

Health Connect America is seeking a Family Development Specialist to support clients and families by developing and implementing treatment plans and linking them to services. This role emphasizes effective communication, documentation, and collaboration with the treatment team.

You will recruit new referrals, participate in weekly Treatment Team meetings, and provide 24/7 on-call support as required by the program, while ensuring compliance with state regulations and COA standards.

Qualifications

• Bachelor's degree in a human services discipline such as Social Work, Psychology, Counseling, Sociology, Criminal Justice, or a related field, is required.
• Minimum of two years experience working with children and/or families.

Responsibilities

• Develop individualized treatment plans and coordinate care for clients and families with input from the treatment team and the Clinical Supervisor.
• Provide individual, group, and family therapeutic interventions, as well as skill-building, outlined in the Treatment Plan.
• Attend and actively participate in weekly Treatment Team meetings.
• Link clients and families with specific services and resources as identified in the treatment plan.
• Serve as a liaison with other professionals, agencies, and community resources related to the client, ensuring their needs are met.
