Job summary

Qualifications

• Bachelor's degree in social work or related field is required.
• One year of social service experience or approved equivalent.
• Familiarity with EBPs such as motivational interviewing and trauma-informed care is preferred.
• Spanish language skills a plus.

Responsibilities

• Conduct initial assessments within 48 hours of arrival.
• Maintain CARES data entry and client chart documentation.
• Develop Independent Living Plans (ILP) with clients and track progress.
• Coordinate referrals to medical, mental health, housing and job training services.
• Provide comprehensive case management including crisis intervention and counseling.
• Attend shelter/agency meetings and complete mandated reporting.
