Qualifications

• Strong organizational and communication skills.

Responsibilities

• Commit to using evidence-based practices (EBP), Motivational Interviewing (MI), Trauma-Informed Care (TIC), and CTI.
• Conduct initial assessments within 48 hours of arrival.
• Initiate referrals for emergency services.
• Enter intake information and maintain CARES data in client charts.
• Provide case management, crisis intervention, and counseling as needed.
• Develop Independent Living Plans (ILP) with client goals.
• Assist clients in obtaining income and public benefits.
• Track referrals and follow up on client progress.
• Attend meetings and participate in training; may work evenings and weekends.
