Job summary

Comunilife Inc. in the Bronx seeks an experienced Case Manager for its transitional residential program Safe Haven II. The Case Manager coordinates comprehensive services, conducts assessments, and develops service plans, ensuring required services are rendered and documented.

Qualifications

• Bachelor's Degree required; with minimum two years field work experience and direct mental illness work experience.
• Associates degree with commensurate case management experience will be considered.
• Bi-lingual, bi-cultural in Spanish is a plus.
• Experience with homeless individuals in congregate programs is a plus.

Responsibilities

• Provide comprehensive case management services to assigned clients and family members.
• Coordinate services across medical, mental health, and social service providers; develop and monitor service plans.
• Conduct home visits or office visits at least weekly and reassess plans quarterly.
• Assist clients in housing transitions and access to treatment, vocational, and support programs.
