Case Manager position in Long Island City, Queens, NY is seeking a professional to manage a caseload of 25 clients in transitional housing. The role requires coordinating housing and employment goals while complying with funder requirements.

The incumbent will perform assessments, referrals, and ongoing case management to support client independence. Responsibilities include intake, CARES data entry, client education, and developing an Independent Living Plan.

Qualifications

Bachelor's degree in social work or a related field is required.
One year of social service experience or approved equivalent.
Familiarity with EBPs such as strength-based case management, motivational interviewing, trauma-informed care, critical time intervention, boundary setting, and cultural reflexivity preferred.
Familiarity with entitlement systems and procedures is preferred.
Experience with standardized assessment tools preferred.
Excellent organizational, written, and verbal communication skills are preferred.
Bilingual English/Spanish proficiency a plus.

Responsabilités

Commits to using evidence-based practices to help clients achieve their goals.
Carry out initial assessments within 48 hours of arrival at the facility.
Initiate referrals for necessary emergency services and intake information.
Maintain CARES data entry and client charts.
Develop Independent Living Plans (ILP) for clients and track progress.
Provide crisis intervention, counseling, and other support services.
Assist clients with income and public benefits and link to community resources.
Attend shelter and agency meetings and training; may work evenings and weekends.
