Qualifications

• Bachelor's degree in social work or related field is required.
• One year of social service experience is required.
• Familiarities with evidence-based practices (EBP) such as strength-based case management, motivational interviewing, trauma-informed care, and related approaches preferred.

Responsibilities

• Commits to using evidence-based practices to help clients achieve their goals.
• Conducts initial assessments within 48 hours of arrival.
• Initiates referrals for emergency services.
• Maintains CARES data entry and chart documentation.
• Provides crisis intervention, counseling, and support services.
• Develops Independent Living Plans for each client.
• Tracks referrals and progress of clients.
