Job summary

Hogar, Inc. is seeking a Case Manager to oversee client services in transitional housing. You will manage a caseload of 25 clients, guiding housing and employment goals while ensuring compliance with funders’ requirements.

You will conduct initial assessments within 48 hours, develop Independent Living Plans, and coordinate referrals to medical, mental health, and education programs. Strong documentation and cross-team collaboration are essential.

Qualifications

• Bachelor's degree in social work or related field required.
• 1 year of social service experience or approved equivalent preferred.
• Familiarity with evidence-based practices (EBP) such as MI, TIC, and CBT preferred.
• Excellent organizational, written, and verbal communication skills preferred.

Responsibilities

• Provide case management for clients in transitional housing with a 25-case caseload.
• Perform initial assessments within 48 hours of arrival; complete intake documentation.
• Develop Independent Living Plans (ILP) and track progress with clients.
• Coordinate referrals to medical, mental health, education/job training, and community resources.
• Maintain CARES data entry and client charts; ensure accuracy and completeness.
• Attend meetings and trainings; work evenings, one Saturday per month and holidays as scheduled.
