Job summary

eBatiz Associates Inc. is seeking a Case Manager in New York to oversee caseloads, conduct initial assessments, and coordinate housing and employment goals for clients in transitional housing. The role emphasizes evidence-based practices, trauma-informed care, and coordinated referrals to community resources.

The ideal candidate will manage a caseload of 25, provide crisis intervention, and ensure documentation in CARES and client charts, with evening and weekend duties as required.

Qualifications

• Bachelor's degree in social work or related field required.
• 1 year of social service experience or equivalent required.
• Experience with E.B.P. and related approaches preferred.
• Bilingual English/Spanish a plus.

Responsibilities

• Provide case management services for clients in transitional housing.
• Conduct initial assessments within 48 hours of arrival.
• Coordinate referrals to medical, mental health, and education/training services.
• Maintain client data in the Uniform Client Chart and CARES documentation.
• Develop and monitor Independent Living Plans for clients.
