Job summary

H.O.G.A.R. is seeking a Case Manager in Astoria, Queens, NY to provide comprehensive case management for clients in transitional housing. The role involves managing an assigned caseload, aiding housing and employment goals, and ensuring compliance with funders' requirements.

The candidate develops living plans, conducts risk assessments, coordinates services, and documents progress in CARES while coordinating with housing specialists and staff. Evening and weekend work is required.

Qualifications

• Must be committed to H.O.G.A.R.'s mission, vision, and values.
• Bachelor's degree in social work or related field required.
• One year of social service experience or approved equivalent.
• Familiarities with EBPs such as strength-based case management and MI preferred.
• Familiarity with entitlement systems and procedures preferred.
• Experience with standardized assessment tools preferred.
• Excellent organizational, written, and verbal communication skills preferred.
• Ability to work effectively in a team environment.
• Computer skills and knowledge of CARES a plus; bilingual English/Spanish a plus.

Responsibilities

• Conduct initial assessment within 48 hours of arrival.
• Develop and monitor Independent Living Plans with clients.
• Provide case management, crisis intervention, and counseling.
• Assist clients with income and public benefits applications.
• Maintain client data in the Uniform Client Chart and CARES system.
• Coordinate referrals to medical, mental health, education, and housing services.
• Track referrals and client progress; ensure regulatory and contractual compliance.
• Prepare mandated reporting and participate in meetings and trainings.
• Work evenings, one Saturday per month, and holidays as scheduled.
