Job summary

Barnes Healthcare Services is seeking a Transitional Care Coordinator to support intake for durable medical equipment and infusion pharmacy patients, while assisting internal sales and ensuring high-quality patient care.

The role emphasizes payer documentation, referrals, and collaboration with case managers to meet organizational objectives and improve patient outcomes. Prior healthcare experience and relevant certifications are preferred.

Qualifications

• High school diploma or equivalent required.
• Two years of clinical healthcare experience, patient intake, and/or medical billing experience preferred.
• Pharmacy technician state licensing preferred.
• Durable medical equipment certification preferred.
• Experience with infusion pharmacy concepts and principles preferred.
• Experience with medication and dosage measurements preferred.

Responsibilities

• Models Barnes culture and acts as a coach and mentor for others.
• Understands payer requirements and secures documentation to facilitate patient care and billing.
• Builds and maintains relationships with case managers and referral partners to support patient care and organizational goals.
• Monitors referral channels and processes referrals promptly to meet payer and patient needs.
• Proactively works with partners to maximize opportunities and drive organizational objectives.
• Processes test claims to determine patient responsibility and remove barriers to care.
• Collaborates with internal and external resources to maximize impact on patient care.
• Participates in on-call schedule as required.
• Other duties as assigned.
