Job summary

Adoration Health is seeking a Care Transition Coordinator to facilitate transitions from healthcare facilities to home health or hospice care. The role involves evaluating patient eligibility, coordinating care plans, and arranging DME/infusion and related services in line with agency protocols.

You will liaise with referral sources, educate patients, and support market growth through outreach and compliant administrative practices.

Qualifications

Minimum of one year of experience in home health or hospital-based case management.
Active licensure in RN, LPN, SW, or PT in the state of practice.
Valid driver’s license and reliable transportation.
Understanding of Medicare/insurance coverage guidelines.

Responsibilities

Achieve monthly production goals and MC admission targets for assigned locations.
Implement strategies to increase market share within assigned facilities.
Evaluate patients and physician orders for home care eligibility according to guidelines.
Coordinate face-to-face patient transitions and educate patients on care plans.
Present patient needs to leadership to obtain branch approval and document encounters in the system.
Coordinate transfer orders and ancillary services (e.g., DME, infusion) and educate patients on services.
Ensure all patient needs identified by referrals are addressed by the agency.
Collaborate with leadership to drive growth aligned with referral sources and patient needs.
Handle sales administration tasks and maintain timely communication via phone and email.
Educate patients on post-discharge physician appointments and medication regimens.
