Job summary

Adoration Health is seeking a Care Transition Coordinator to facilitate patient transitions from facilities to home health or hospice. The role evaluates eligibility, coordinates care plans, and ensures services such as DME and infusion are arranged per agency procedures.

The CTC liaises with the agency, referral sources, and healthcare providers to ensure timely communication, documentation, and patient education, while supporting market growth and compliance with financial stewardship.

Qualifications

• Must have active licensure in the state of practice as RN/LPN/SW/PT
• Experience in home health or hospital-based case management preferred
• Strong communication and organizational skills required

Responsibilities

• Achieve monthly production goals and MC admission targets.
• Coordinate care plans and assist with DME and infusion arrangements.
• Evaluate eligibility for home care per Right of Choice guidelines.
• Educate patients and coordinate with physicians and executive leadership.
• Document encounters and coordinate transfer orders for patient care.
• Educate medical professionals about agency services and Medicare guidelines.
