Job summary

Enhabit Home Health & Hospice is seeking a qualified clinical reviewer to evaluate patient information across start of care, recertification and evaluation visits. You will educate LPs, ensure documentation accuracy and maintain relationships with referral sources, patients and caregivers.

Ideal candidates have nursing or physical therapy credentials, a valid license, and CPR certification, with preferred Medicare home health or hospice experience.

Qualifications

• Graduate of an approved school of professional nursing or physical therapy.
• Maintain a valid clinical license as required by state rules.
• Minimum one year of experience; Medicare home health or hospice setting preferred.
• CPR certification and valid state driver license required.
• Automobile liability insurance as required by law.
• Proficient with basic technology and Microsoft Office applications.

Responsibilities

• Review and approve patient information submitted by the Licensed Professional (LP) at start of care, recertification, resumption of care or evaluation visits.
• Partner with branch leadership to educate and train LPs on documentation and regulatory requirements.
• Provide one-on-one training when necessary and maintain relationships with referral sources, patients, caregivers and staff.
• Promote quality patient care and ensure staff understand updates to procedures and standards.
