Job summary

Enhabit Home Health & Hospice is seeking a licensed professional to review and approve patient information during start of care, recertification, resumption of care, or evaluation visits. You will collaborate with branch leadership to provide ongoing education and ensure documentation meets regulatory standards.

Responsibilities include training LPs, promoting quality patient care, and maintaining strong relationships with referral sources, patients, caregivers and staff.

Qualifications

• Must be graduate of an approved school of professional nursing or physical therapy.
• Current valid clinical license as per state requirements.
• Minimum one year of experience; Medicare home health or hospice setting preferred.
• CPR certification required.
• Valid state driver license and automobile liability insurance.
• Basic technology skills including Microsoft Office and mobile devices.
• Experience with Homecare Homebase (HCHB) preferred.

Responsibilities

• Review and approve all patient information submitted during start of care, recertification, resumption of care, or evaluation visit.
• Partner with branch leadership to educate LPs and ensure documentation meets regulatory standards.
• Provide in-depth training to individual LPs as needed.
• Promote quality patient care and maintain relationships with referral sources, patients, caregivers and employees.
