Ambercare – an Addus family company is seeking a Service Coordinator in Truth or Consequences, NM. This role will schedule caregivers, supervise in-home staff, and ensure high-quality home care services while maintaining client records and care plans.

Based in an in-office setting, the coordinator will handle client updates, staff coordination, and monthly wellness calls, contributing to reliable, compassionate care for adults with disabilities and the elderly.

Qualifications

• High school diploma or equivalent.
• 1 year of industry experience required.
• Interpersonal, organizational and communication skills.
• Computer skills including Microsoft Word and Excel and scheduling program.
• Must have reliable transportation.

Responsibilities

• Creates and manages in-home caregiver schedules and updates.
• Provides coverage to ensure care plans are followed and uninterrupted.
• Contacts care providers and clients for service updates.
• Conducts monthly client wellness calls and home visits as needed.
• Monitors escalated client complaints and resolves issues.
• Supervises direct service employees and enforces policy adherence.
