Qualifications

• 2+ years experience in senior services or related social services field.
• Excellent oral and written communication skills.
• Strong interpersonal skills and customer service mindset.

Responsibilities

• Create and manage work schedules for in-home care workers.
• Conduct in-home visits to monitor care plans and services.
• Coordinate with clients and providers to ensure timely service delivery.
• Ensure compliance with Medicare, Medicaid, and HIPAA regulations.
• Maintain accurate evaluation reports and case notes.
