AmbercareAddus HomeCare in Espanola, NM is seeking a Service Coordinator to schedule caregivers, support field staff, and keep care plans on track. You will lead scheduling, conduct in-home visits, and act as a problem-solver in a fast-paced environment.

The role is full-time, in office, Monday–Friday 8am–5pm, with opportunities to coordinate across a geographic area and maintain client wellness through regular outreach.

Qualifications

• Must have a high school diploma or equivalent.
• 1 year of industry experience required.
• Supervisory and/or home care experience preferred.
• Interpersonal organizational and communication skills; Computer skills including Microsoft Word, Excel, and Scheduling program.

Responsibilities

• Creates work schedules by entering schedules into the system and manages changes to client schedules.
• Provides alternate coverage to ensure the client care plan is followed and services are not interrupted.
• Contacts care providers and clients to provide service updates.
• Conducts monthly client wellness calls and home visits as required.
• Escalates client complaints and theft claims and ensures follow-through.
• Supervises direct service employees by setting expectations and enforcing policies.
• Maintains confidentiality and a professional, organized branch environment.
• Follows applicable regulations and company standards.
