Ambercare – an Addus family company seeks a Full Time Service Coordinator in Clovis, NM. This role is responsible for scheduling and supervising in-home care workers and clients within the service area, including conducting home visits to monitor care plans and ensure service quality, with opportunities for growth and agency bonuses.

Qualifications

• Minimum of 2 years’ experience in senior services or related social services.
• Excellent oral and written communication skills.
• Strong interpersonal skills.
• Ability to prioritize and handle high call volumes.
• Client-focused and customer service-driven.
• Proficient in Microsoft Word, Excel, and Outlook.
• Must have reliable transportation, valid driver’s license and insurance for travel within the branch service area.

Responsibilities

• Creates and manages staff schedules; adjusts per client needs.
• Provides alternate coverage to ensure care plans are followed.
• Contacts care providers and clients to provide service updates.
• Monitors client conditions and Home Care Aides performance.
• Maintains compliance with Medicare, Medicaid, and HIPAA rules.
• Conducts home visits to welcome new clients and review Welcome Packet.
• Prepares evaluation reports and case notes; maintains schedules for timely visits.
• Attends weekly branch meetings and maintains confidentiality and attendance.
• Performs other duties as assigned.
