Ambercare is seeking a Full Time Service Coordinator in Albuquerque, NM to schedule and supervise in-home care workers and clients in a defined geographic area.

The role includes conducting in-home visits to monitor care quality, adjust care plans as needed, and ensure adherence to regulatory standards. Requires strong communication, interpersonal skills and 2+ years in senior services.

Qualifications

• Two years’ experience in senior services or related social service field.
• Scheduling and/or case management experience is preferred.
• Excellent oral and written communication skills are essential.
• Strong interpersonal and customer service orientation.
• Proficient with MS Office and ability to manage travel within branch area.

Responsibilities

• Create and manage home care worker and client schedules.
• Supervise in-home visits and monitor service quality.
• Assess and adjust care plans to meet client needs.
• Conduct welcome visits and review care packets with new clients.
• Ensure compliance with Medicare, Medicaid, HIPAA and company policies.
• Coordinate coverage to maintain uninterrupted client care.
• Document evaluations and case notes and report to supervisor.
• Attend weekly branch meetings and maintain confidentiality.
