Job summary

Ambercare/Addus HomeCare is seeking a Service Coordinator to lead scheduling of in-home caregivers and oversee care plans across a geographic area. You will coordinate client updates, supervise field staff, and ensure continuity of service in a fast-paced environment.

Previous home care supervisory experience is preferred; you will be the go-to problem solver ensuring quality, compassionate care for older adults and adults with disabilities in their homes.

Qualifications

• Must have high school diploma or equivalent.
• 1 year of Industry experience required
• Interpersonal, organizational and communication skills.
• Computer skills including but not limited to Microsoft Word, Microsoft Excel and Scheduling program.
• Must have reliable transportation.

Responsibilities

• Creates and updates work schedules in the system, adjusts for client needs, illness, vacations.
• Provides alternate coverage to ensure the client’s care plan is followed and client services are not interrupted.
• Contacts care providers and clients to provide service updates.
• Conducts monthly client wellness calls and conducts home visits as required.
• Provides follow-through on escalated client complaints and theft claims.
• Supervises direct service employees by setting expectations and enforcing policies.
• Maintains a professional, neat, clean, and clutter-free branch environment.
• Maintains confidentiality due to access to sensitive information.
• Maintains regular, predictable attendance and flexibility to meet agency needs.
• Follows all MCO, Medicare, Medicaid, and HIPAA regulations.
• Abides by all regulations, policies, procedures and standards.
