Ambercare/Addus HomeCare is seeking a Full Time Service Coordinator in Clovis, NM. The role involves scheduling and supervising in-home care workers and clients across a geographic area, with regular home visits to monitor and adjust care plans to meet client needs.

Responsibilities include coordinating staff, ensuring continuity of care, and reporting on service quality, while complying with Medicare/Medicaid/HIPAA regulations. Local travel within the branch area is required.

Qualifications

• Minimum of 2 years’ experience in senior services or related social services.
• Excellent oral and written communication skills.
• Strong interpersonal skills.
• Ability to prioritize and handle high call volumes.
• Client-focused and customer service driven.
• Proficient in Microsoft Word, Excel, and Outlook.
• Must have reliable transportation, valid driver’s license and insurance for travel within the branch service area.

Responsibilities

• Creates and manages work schedules for Home Care Aides and clients.
• Ensures care plans are followed and coverage is provided.
• Conducts home visits to welcome new clients and review Welcome Packet.
• Monitors client condition and aides performance.
• Maintains compliance with Medicare, Medicaid, HIPAA regulations.
• Attends weekly branch meetings and handles case notes.
