Addus HomeCare in Truth or Consequences, NM is seeking a driven Service Coordinator to lead scheduling of caregivers, supervise in-home visits, and ensure care plans stay on track. This role combines problem solving with strong organization to support clients and field staff in a fast-paced home care environment.

You will create and adjust schedules, coordinate coverage, perform wellness calls, document updates, and maintain compliance with Medicare/Medicaid/HIPAA.

Qualifications

• Requires 1 year of industry experience.
• Strong communication and organizational abilities.
• Must have reliable transportation.

Responsibilities

• Schedule and supervise in-home care workers and clients.
• Provide alternate coverage to ensure continuous care.
• Conduct monthly wellness calls and home visits as required.
• Document updates and escalated client issues; ensure regulatory compliance.
