Job summary

Care Lync is seeking a dedicated Support Coordinator / Service Coordinator / Case Manager to develop individualized care management plans and coordinate services for adults with IDD, autism, and physical disabilities.

You will broker services, monitor outcomes, and support clients in living independently, including housing, medical, social, and vocational supports, with up to 50% travel as needed.

Qualifications

• BS/BA degree in related course work.
• Minimum of 3 years related experience providing case management for individuals with Intellectual Disabilities, Autism Spectrum Disorders, or Physical Disabilities.
• Experience with Microsoft Office, state-maintained databases.
• CPR/First Aid certification and valid driver's license with vehicle insurance coverage.
• Inform supervisor immediately when any change in auto insurance/policy, vehicle registration or license status.

Responsibilities

• Complete assessments to identify each participant's needs and desires for service delivery.
• Develop a Person-Centered Service Plan for each participant and update annually or with significant changes.
• Inform participants about provider options to help them make fully informed decisions.
• Ensure Behavioral/ Crisis Plans align with the ISP when applicable.
• Report all incidents of abuse, neglect or exploitation immediately to supervisor.
• Coordinate with other care coordinators and entities to ensure seamless service delivery.
• Research and develop alternative solutions to participant needs.
• Maintain confidentiality of sensitive personal and health information.
• Attend required trainings and recertification classes.
• Regularly discuss with supervisor to understand participant needs and capabilities.
• Travel to participants as needed (approximately 50%).
