Job summary

Care Lync is seeking a dedicated Support Coordinator/Service Coordinator/Case Manager to perform comprehensive case management for adults with Intellectual Disabilities, Autism Spectrum Disorder, and Physical Disabilities. You will build individualized care plans, coordinate services, and monitor outcomes to ensure cost-effective, high-quality support in home and community settings.

The role requires 3+ years of experience, proficiency in Microsoft Office, and current CPR/First Aid

Qualifications

• BS/BA degree in related coursework.
• Minimum of 3 years related experience providing case management for individuals with Intellectual Disabilities, Autism Spectrum Disorders, or Physical Disabilities.
• Experience with Microsoft Office, state-maintained databases.
• Must maintain proficiency in company sponsored training and certifications.
• Maintain CPR/First Aid certification and updated state related Clearances.
• Valid Driver's License including personal vehicle insurance coverage for liability and current vehicle registration.
• Inform supervisor immediately when any change in auto insurance policy or status of license.

Responsibilities

• Complete assessments to identify participant needs and desires for service delivery.
• Develops a Person-Centered Service Plan for each participant and modify at least annually or with significant changes.
• Ensure participant choice of providers by providing information for fully informed decisions.
• Ensure Behavioral Support Plan and Crisis Plan align with the ISP.
• Report all incidents of participant abuse, neglect or exploitation immediately.
• Actively coordinate with other care coordinators and entities to ensure seamless service delivery.
• Research and develop alternative solutions to participants' needs.
• Maintain confidentiality of sensitive material and private health information.
• Attend required trainings and recertification classes.
• Consult with supervisor(s) to consider participant needs and limitations.
• Travel up to 50% to participants as needed.

Education

BS/BA degree in related course work

3+ years case management experience

Microsoft Office proficiency

Maintain CPR/First Aid certification

Clean state clearances

Valid driver’s license & vehicle insurance
