Job summary

Care Lync is seeking a dedicated Support Coordinator to empower adults with disabilities and aging individuals to live independently. You will conduct person-centered assessments, create and update ISPs, and advocate for participants while coordinating with healthcare professionals and family members to secure appropriate services.

The role supports community integration, maintains confidentiality, and requires up to 50% travel to participants’ homes and communities.

Qualifications

• Bachelor's degree in Social Work, Psychology, Human Services, or related field.
• Strong organizational, communication, and advocacy skills.
• Comfort working independently and traveling (up to 50%) to meet with participants in their homes and communities.
• Valid driver's license, auto insurance, and reliable transportation.
• Commitment to maintaining required certifications and state clearances.

Responsibilities

• Conduct person-centered assessments to identify participants' needs, preferences, and life goals.
• Develop, implement, and update Individual Support Plans (ISP) to ensure participants receive the right services.
• Provide participants with choice and advocacy, helping them make informed decisions about providers and care options.
• Coordinate with medical professionals, behavioral specialists, family members, and community resources to deliver high-quality care.
• Monitor and evaluate services to ensure safety, satisfaction, and progress toward independence.
• Support individuals with disabilities and aging population in accessing community resources.
• Maintain documentation, confidentiality, and compliance with program and ethical standards.
• Participate in ongoing training and professional development.
