Job summary

Care Lync is seeking a dedicated Support Coordinator to develop and manage individualized care plans for adults with IDD, autism, and physical disabilities. Responsibilities include coordinating services, monitoring outcomes, and ensuring confidentiality while advocating for participant needs.

The role requires a BS/BA in a related field with at least 3 years of case management experience, proficiency in Microsoft Office, and a valid driver’s license for travel to clients.

Qualifications

• BS/BA degree in related field or equivalent experience.
• Minimum of 3 years related experience in case management for individuals with IDD, autism, or physical disabilities.
• Experience with Microsoft Office and managing state databases.
• Maintain CPR/First Aid certification and required trainings.
• Valid driver’s license and access to a vehicle for client travel.

Responsibilities

• Complete assessments to identify each participant's needs and preferences for service delivery.
• Develop a Person-Centered Service Plan and update it at least annually.
• Present provider options to participants to enable informed choices.
• Coordinate with care teams to ensure seamless delivery of physical, behavioral, and support services.
• Travel to participants as needed (up to 50%).
