Job summary

Care Lync is seeking a dedicated Support Coordinator to manage casework for adults with Intellectual, Developmental, and Physical Disabilities. You will develop individualized care plans, coordinate services, and monitor outcomes to ensure high-quality, cost-effective supports for clients in their homes and communities.

The role involves identifying client needs, coordinating with providers, and ensuring service continuity.

Qualifications

Minimum of 3 years related experience providing case management for individuals with Intellectual Disabilities, Autism Spectrum Disorders, or Physical Disabilities.
Experience with Microsoft Office, state-maintained databases.
Maintain CPR/First Aid certification and updated state related Clearances.
Valid Driver's License including personal vehicle insurance coverage for liability and current vehicle registration.

Responsibilities

Complete assessments to identify each participant's needs and desires for service delivery.
Develop a Person-Centered Service Plan for each participant and modify annually or with changes.
Ensure participant choice of providers by providing information for informed decisions.
Coordinate with other care coordinators to ensure seamless service delivery.
Travel up to 50% to participants as needed.
