Job summary

Care Lync is seeking a dedicated Support Coordinator to develop individualized care plans and coordinate services for adults with IDD, autism, and physical disabilities under DHS waivers.

You will assess needs, arrange services, monitor outcomes, and advocate for participant-centered care while safeguarding confidentiality. Expect up to 50% travel to support participants in home and community settings.

Qualifications

• BS/BA degree required and related field experience.
• Minimum of 3 years related case management experience for individuals with IDD, autism, or physical disabilities.
• Experience with Microsoft Office and state-maintained databases.
• Maintain CPR/First Aid certification and updated state clearances.
• Valid Driver's License including personal vehicle insurance and registration.

Responsibilities

• Complete assessments to identify participant needs and service desires.
• Develop a Person-Centered Service Plan and revise annually or with significant changes.
• Provide information for fully informed provider choices.
• Coordinate with Behavioral/Support plans to align with ISP.
• Report incidents of abuse, neglect, or exploitation immediately.
• Coordinate with other care coordinators to ensure seamless service delivery.
• Develop alternative solutions to participant needs.
• Ensure confidentiality of sensitive health information.
• Attend trainings and recertification classes.
• Travel to participants as needed (approx. 50%).
