You will guide health, psychological, and independence goals through interviews and team collaboration while ensuring safe, compliant residential operations.

Qualifications

• Experience coordinating with an interdisciplinary service team.
• Ability to identify and communicate service needs through interviews.
• Experience writing person-centered plans and monitoring progress.

Responsibilities

• Coordinating IDT meetings and the ISPs.
• Assisting people to identify and communicate needs for services via interviews.
• Supervising and coordinating person-centered plans and monitoring outcomes.
• Initiating periodic reviews and adjustments based on feedback.
• Reviewing assessments to help individuals meet potential goals.
• Overseeing residential activity to ensure health and safety.
• Visiting centers and homes to assess plan implementation.
• Communicating with Administrator and colleagues to improve workflows.
• Ensuring QA and state survey improvements are addressed.
• Linking people with families/guardians for service approvals.
