Job summary

RHA Health Services is seeking an organized Qualified Professional to coordinate and monitor person-centered plans and support services for individuals with intellectual and developmental disabilities and mental health needs. You will supervise team members, complete assessments, and help implement individualized plans across residential and vocational settings.

The role emphasizes strong communication, adherence to regulations, and the ability to lift up to 50 pounds, with CPR proficiency.

Qualifications

• Bachelor’s degree in a human service field and two years of experience, or four years in other fields with four years of experience, or a license in a human service profession.

Responsibilities

• Serve as the center of the interdisciplinary service team to guide development of person-centered plans.
• Complete incident reports and employee performance evaluations.
• Supervise and write person-centered plans based on assessments and interviews.
• Oversee residential activities, home appearance, cleanliness and community integration.
