Job summary

RHA Health Services, LLC is hiring an IDD Qualified Professional (Regular) to support person-centered plans for individuals with intellectual and developmental disabilities and mental health challenges. This role coordinates interdisciplinary team efforts, ensures clinical and functional outcomes, and communicates with families and guardians to obtain service approvals.

Qualifications

• Bachelor’s degree in Social Work, Social Services, Human Services or Special Education required.
• Master’s degree a plus
• Minimum 2 years experience working with persons with intellectual and/or developmental disabilities required.
• 1-year experience if paired with Master’s degree
• Valid driver’s license, auto insurance, and reliable transportation
• Supervisory/management experience and experience writing person-centered (individualized service) plans preferred
• Must be at least 18 years of age.

Responsibilities

• Coordinating IDT meetings and the ISPs
• Assisting each person we support to identify and communicate needs through direct interviews
• Supervising, integrating and coordinating person-centered plans and monitoring progress
• Initiating periodic reviews, investigations, modifications and adjustments by soliciting feedback
• Reviewing assessments completed by the people we support and other team members
• Supervising and writing the person-centered plan based on assessments, interviews, and observations
• Overseeing residential activity: home appearance, materials, cleanliness, community integration
• Visiting vocational centers and other service areas to assess plan implementation
• Reporting to the Administrator and coordinating with colleagues to improve procedures
• Monitoring quality assurance and state survey improvements
• Serving as the link with families and guardians for approvals
• All other duties as assigned
