Job summary

RHA Health Services, LLC is hiring a Qualified Professional (QP) for the IDD IDT within an ICF setting. You will coordinate and monitor person-centered services, develop PCPs on admission, and drive successful transitions while maintaining full bed occupancy.

The role involves collaboration with nursing, DSPs, clinical staff, administrators, and guardians to ensure regulatory compliance and person-centered outcomes. Strong communication and organization are essential.

Qualifications

• Bachelor’s degree in Social Work, Human Services, Psychology, Special Education, or related field (Master’s degree preferred).
• At least 2 years experience (NC) or 1 year (TN); 1 year required if Master’s degree is held.
• Experience in developing, writing, and implementing Person-Centered Plans (PCPs).
• Working knowledge of ICF regulations, PCP principles, and Medicaid/HCBS rules.
• Excellent communication, organizational, and documentation skills.
• Valid driver’s license, auto insurance, and reliable transportation.
• Must be at least 18 years of age.

Responsibilities

• Admissions & Census Management: lead intake and admission, PCP on admission, ensure bed occupancy.
• Service Coordination: coordinate IDT meetings, translate needs into PCP goals, supervise PCP implementation.
• Monitoring & Oversight: site visits to assess PCP fidelity, safety, and community integration.
• Advocacy & Communication: engage families/guardians, advocate for individuals’ rights.
• Other Duties: maintain documentation per Medicaid/ICF, participate in trainings and reviews.
