Job summary

RHA Health Services is seeking a Qualified Professional to coordinate and monitor services for individuals with intellectual and developmental disabilities and mental health needs. The role centers on interviewing people we serve, developing person-centered plans, and coordinating with nurses, direct care staff, and administrators.

The ideal candidate will hold a social work or human services degree (Master’s preferred), possess strong communication skills, and have experience with regulatory

Qualifications

• Bachelor’s degree in Social Work, Social Services, Human Services or Special Education is required; Master’s degree a plus.
• Minimum 2 years experience working with persons with intellectual and/or developmental disabilities; 1-year if paired with Master’s degree.
• Working knowledge of applicable regulations that apply to our service offerings.

Responsibilities

• Guide development of person-centered plans and overall programs based on interviews and assessments.
• Complete incident reports and employee performance evaluations.
• Supervise and coordinate person-centered plans and monitor progress toward outcomes.
• Review assessments to help people meet their full potential.
• Oversee residential activities to ensure health and safety.
