Job summary

RHA Health Services in North Carolina is seeking an IDD Qualified Professional ICF to coordinate, monitor, and support person-centered services for individuals with IDD. You will develop PCPs, manage admissions, and help maintain full bed occupancy within the facility.

As a key member of the IDT, you will collaborate with nursing, direct support staff, clinicians, and guardians to align services with each person’s goals and preferences, ensuring regulatory compliance and ongoing quality

Qualifications

• Bachelor’s degree in Social Work, Human Services, Psychology, Special Education, or related field; Master’s preferred.
• Minimum 2 years’ experience in NC; 1 year in TN if Master’s.
• Experience developing, writing, and implementing Person-Centered Plans.
• Knowledge of ICF, Medicaid/HCBS rules.
• Strong communication, organizational, and documentation skills.
• Valid driver’s license and reliable transportation.
• Must be at least 18 years old.

Responsibilities

• Lead intake and admission processes and PCP development with the IDT.
• Coordinate all aspects of admission and ensure timely documentation.
• Develop initial PCP upon admission with IDT collaboration.
• Monitor bed occupancy and support referral development and follow-up.
