Job summary

RHA Health Services in North Carolina is seeking an IDD Qualified Professional for the ICF program. You will coordinate person-centered plans, oversee admissions, and support transitions to ensure high occupancy and quality care.

Work with the interdisciplinary team, guardians, and families to align services with individual goals, promote independence, and regulatory compliance through PCP development and ongoing supervision.

Qualifications

• Bachelor’s degree in Social Work, Human Services, Psychology, Special Education, or a related field.
• Minimum 2 years in NC; 1 year in TN with IDD or related populations.
• Experience developing and implementing Person-Centered Plans and knowledge of ICF regulations.

Responsibilities

• Lead intake, admissions, and PCP development upon admission.
• Coordinate IDT meetings and monitor PCP fidelity and outcomes.
• Conduct site visits to evaluate PCP implementation and environment.
• Maintain documentation per Medicaid, ICF, and company requirements.
• Provide advocacy and communications with families and guardians.
