Job summary

RHA Health Services, LLC seeks a Qualified Professional to join its IDDT team, coordinating person-centered plans and admissions within an ICF setting. The role emphasizes collaboration with nursing, direct support staff, clinicians, and guardians to drive outcomes and regulatory compliance.

The position requires a Bachelor’s in social/human services related field (MS preferred), 2+ years in NC or 1 year in TN with IDD, and strong documentation skills. Travel for site visits may be required.

Qualifications

• Bachelor’s degree in Social Work, Human Services, Psychology, Special Education, or related field (Master’s degree preferred).
• At least 2 years of experience in North Carolina, 1 year in Tennessee with individuals with IDD; 1 year required if Master’s degree is held.
• Experience developing, writing, and implementing Person-Centered Plans.
• Working knowledge of ICF regulations, person-centered planning principles, and Medicaid/HCBS rules.
• Excellent communication, organizational, and documentation skills.

Responsibilities

• Lead admissions and census management, including pre-admission screenings and coordination of transitions into the facility.
• Develop and implement PCPs in collaboration with the IDT and ensure PCP fidelity and progress toward outcomes.
• Coordinate IDT meetings, interviews, and ensure documentation meets regulatory timelines.
• Conduct site visits to evaluate PCP implementation and identify opportunities for improvement.
• Maintain communication with families, guardians, and external stakeholders about planning and delivery of services.
• Maintain timely and accurate documentation per Medicaid, ICF, and company requirements.
• Participate in ongoing training, compliance reviews, and team meetings.
