Job summary

RHA Health Services, LLC in Kernersville, NC seeks an IDD Qualified Professional to coordinate person-centered services in an Intermediate Care Facility. You will lead admissions, develop PCPs, and help maintain full bed occupancy through referrals and intake.

As primary care coordinator, you will collaborate with nursing, direct support staff, clinical specialists, and guardians to ensure goals, independence, and regulatory compliance.

Qualifications

• Bachelor’s degree in Social Work, Human Services, Psychology, Special Education, or related field required (Master’s preferred).
• 2 years of NC experience, 1 year in Tennessee; 1 year required if Master’s.
• Experience developing, writing, and implementing Person-Centered Plans.

Responsibilities

• Lead admissions and census management, including pre-admission screenings and PCP development.
• Coordinate IDT meetings and ensure timely documentation and regulatory compliance.
• Monitor PCP implementation and progress toward outcomes.
• Maintain communication with families, guardians, and stakeholders; advocate for individuals.
