Job summary

RHA Health Services, LLC in Greensboro, NC is seeking an IDD Qualified Professional to coordinate and monitor person-centered services in our Intermediate Care Facility. You will develop and implement PCPs, manage admissions and census, and lead referrals with the IDT and guardians to ensure smooth transitions and timely placement.

As primary coordinator of care, you will translate assessments into actionable goals, monitor service delivery for fidelity to PCPs, and advocate for individuals

Qualifications

• Bachelor's degree in Social Work, Human Services, Psychology, Special Education, or a related field.
• Master's degree preferred.
• 2 years of experience with IDD in NC; 1 year in TN.
• Experience developing Person-Centered Plans; knowledge of ICF Medicaid/HCBS rules.
• Valid driver's license and reliable transportation.

Responsibilities

• Lead intake and admissions process, including pre-admission screenings and PCP creation.
• Coordinate the IDT meetings and ensure PCP fidelity and outcomes.
• Conduct site visits to monitor service delivery, safety, and community integration.
• Maintain documentation per Medicaid/ICF requirements and communicate with guardians.
