Job summary

RHA Health Services seeks a Qualified Professional ICF to coordinate, monitor, and support person-centered services within an IDD/ICF setting. The role focuses on PCP development, admissions, and active occupancy management, while collaborating with nursing, support staff, and guardians to ensure coordinated care.

You will drive person-centered planning, oversee PCP fidelity, and work with the IDT to align services with individual goals and regulatory requirements.

Qualifications

• Bachelor’s degree in Social Work, Human Services, Psychology, Special Education, or a related field (Master’s degree preferred).
• 2+ years of experience with IDD; 1 year in North Carolina, 1 year in Tennessee; 1 year required if Master’s degree is held.
• Demonstrated experience in developing, writing, and implementing Person-Centered Plans.
• Working knowledge of ICF regulations, person-centered planning principles, and Medicaid/HCBS rules.
• Excellent communication, organizational, and documentation skills.
• Valid driver’s license, auto insurance, and reliable transportation.
• Must be at least 18 years of age.

Responsibilities

• Lead Admissions & Census Management: manage intake, pre-admission screenings, and PCPs upon admission.
• Coordinate the IDT meetings and ensure PCP fidelity and progress of outcomes.
• Conduct site visits to evaluate PCP implementation and safety across locations.
• Maintain communication with families and guardians to inform planning and delivery.
• Complete documentation per Medicaid/ICF requirements and participate in training.
