Job summary

RHA Health Services in Greensboro, NC seeks an IDD Qualified Professional to coordinate, monitor, and support person-centered services for individuals with IDD in our Intermediate Care Facility. You will lead PCP development, manage admissions, and drive occupancy by guiding referrals and transitions.

This role emphasizes collaboration with nursing, direct support, clinicians, and guardians to ensure independence, well-being, and compliance with regulators.

Qualifications

• Bachelor's degree in Social Work, Human Services, Psychology, Special Education, or related field; Master’s degree preferred.
• At least 2 years of NC experience and 1 year in TN working with individuals with IDD; 1 year required if Master’s.
• Experience developing, writing, and implementing Person-Centered Plans (PCPs).
• Working knowledge of ICF regulations, person-centered planning principles, and Medicaid/HCBS rules.
• Excellent communication, organizational, and documentation skills.
• Valid driver’s license, auto insurance, and reliable transportation.
• Must be at least 18 years of age.

Responsibilities

• Lead admissions and census management, including PCP development upon admission.
• Coordinate the IDT meetings, ensure PCP fidelity, and document plans.
• Monitor delivery of services to align with PCP and progress toward outcomes.
• Conduct regular site visits to evaluate PCP implementation and safety.
• Maintain timely Medicaid/ICF documentation and compliance.
• Communicate with families and guardians to keep them informed and engaged.
