Job summary

00 RHA Health Services, LLC is hiring a Qualified Professional (QP) to join the IDT within the ICF in North Carolina. You will coordinate person-centered services, develop PCPs, and oversee admissions to maintain full bed occupancy.

The role requires a Bachelor's degree in a related field and IDD experience, with emphasis on collaboration among nursing, staff, guardians, and regulatory compliance.

Qualifications

• Bachelor's degree in Social Work, Human Services, Psychology, Special Education, or a related field (Master's degree preferred).
• At least 2 years of experience (in North Carolina), 1 year of experience (in Tennessee) working with individuals with IDD; 1 year required if Master's degree is held.
• Demonstrated experience in developing, writing, and implementing Person-Centered Plans.
• Working knowledge of ICF regulations, person-centered planning principles, and applicable Medicaid/HCBS rules.
• Excellent communication, organizational, and documentation skills.
• Valid driver's license, auto insurance, and reliable transportation.
• Must be at least 18 years of age.

Responsibilities

• Lead the intake and admission process, including pre-admission screenings, meet-and-greet visits with prospective individuals and their support teams, and determining program compatibility.
• Coordinate all aspects of the admission process, ensuring timely collection of required documentation and smooth transitions into the facility.
• Develop and implement the initial Person-Centered Plan (PCP) upon admission, in collaboration with the IDT.
• Maintain awareness of vacant beds and actively support referral development and follow-up efforts to ensure timely occupancy.
• Serve as the primary coordinator of the IDT, ensuring that interdisciplinary team meetings are scheduled, facilitated, and documented.
